What to expect
Please bring any records and recent X-rays from previous Dentists. Also, please bring your filled out medical history and patient registration forms. You can download these forms from our website or we can mail them to you for your convenience. To have the forms mailed to you please call our office and speak with Chelle or Lisa (887-1177).
