Insurance and Billing
We accept most traditional insurance plans. As a courtesy, we are glad to file insurance claims. In order to process your insurance claims, we need your insurance card and the subscriber’s identification number, many times this is the social security number.

Payment Options
For a mutually beneficial arrangement we offer a 5% cash discount for pre-payment or for payment at time of service.
We also offer Care Credit, a private medical dental financing group that can offer 12 month interest-free credit arrangements. Please contact Lisa with any questions (360-887-1177).
